Hello!

This information is for those of you who are hospitalized and who may need support from Ystad Municipality after your discharge.

The hospital will notify the municipality when the doctor has assessed that you are ready to be discharged and leave the hospital. The municipality will then contact you by telephone if it appears that you may need support from the municipality after your discharge.

The municipality has a team consisting of a social worker, a nurse and an occupational or physiotherapist. This team will contact you if it becomes apparent that you need new or more support at home. During the conversation, we will tell you what we in the municipality can help with and you will be given the opportunity to tell us what support you think you need from the municipality when you leave the hospital. Together we will plan the support you need to get home as soon as possible and in a safe way.
